Location is coarse, not a street address
The location column normally stops at a city or region, taken from the network exit point. A different city does not by itself mean a breach, but a region you have never used is worth a second look.
Read the device column closely
One phone using two different browsers can appear as two entries. Check the browser name and OS version rather than counting devices.

Does a strange location in my login history mean my account was hacked?
